STD Wizard
Prior to your clinic visit, it may be helpful to complete this five-minute questionnaire. The responses to this short questionnaire recommend what specific sexually transmitted diseases you should be tested for. At the end of the questionnaire, you will have the option of printing your results. Please print and bring the results with you to a visit with a health care provider or the McLean County Health Department Personal Health Clinic.
