Directly across the street from the McLean County Government Center 

PCR testing available using self-collected samples done on-site
Walk-up appointments are welcome, and your registration can be done on site


Pre-register by choosing your test and following the link & instructions:

Option 1: To register for PCR nasal-swab testing (age 5 and older) CLICK HERE

NOTE: No username or password is required for nasal swab test appointment. However, you will need to create a login (username and password) to access your test results. See instructions below under “How will I receive my test results?”

How will I receive my test result?

For PCR nasal-swab testing

You have 2 options to access your lab result:

You will be receiving an email after you register for your COVID-19 test. If you pre-register online, you might receive this email before your appointment. If you are a “walk-in” appointment, the staff will register when you arrive (using the email address you provide to them) and you will receive this email shortly after you get your COVID test.

Option #1: Check results yourself using the online portal. 

  • Check your email account (you may need to check your spam folder) and use that email to access the Midwest Coordinating Center portal. 
  • Create a private login. This link provides helpful instructions: CLICK HERE.
  • On average, you will be able to privately access your lab results from this portal 48-72 hours after your test.  

Option #2: No email address? During your COVID-19 test appointment, you can request to receive a phone call from MCHD Staff with your test results.

  • On average, you will receive a phone call within 48-72 hours after your COVID-19 test. You may not recognize the number. 
  • If you do not pick up, the caller will leave a voice message asking only that you return the phone call. You must return the call during regular business hours (Mon-Fri 8am-4pm) and speak to someone to confirm your test results.
  • You WILL NOT receive a text message.

saliva-based testing

Test results can only be accessed online. Check results using the online portal by setting up a profile.

The same website can be used to register for your test and to check your test results:

  • Go to
  • Use Agency Code df5brbrj
  • Choose a test location and click “get tested” (blue box). This leads you to the Login/Register page:
    • If you have already registered, login using your username and password to access your profile.
    • If you have not already registered, create a profile to access test results when they are available.
  • After you have completed the login process, choose what you want to do next:
    • Schedule, view or cancel an appointment
    • View my lab results
  • On average, you will be able to privately access your lab results from this portal 48-72 hours after your test.

Anyone under 18 years old must have a parent or guardian present 
to receive a COVID-19 test.

If the parent assigns another adult to bring their child to the vaccination center, the parent must have completed a proxy form and the completed form must be brought to the clinic by the proxy.
