How does the CDC define being “fully vaccinated” 
and being “up to date” on COVID-19 vaccines?

All residents age 6 months and older are currently eligible.

  • Anyone under 18 years old must have a parent or guardian present to receive a vaccine.
  • If the parent assigns another adult to bring their child to the vaccine appointment, the parent must have completed a proxy form, and the completed form must be brought to the appointment by the proxy.
